  Usually, but I occasionally see "tokens and no data" as well.

  'cvs -q update -PdA' with $cwd being the top of the tree.

  I havn't found one yet.  I thought that repeated rapid writes to a file 
  might cause it (such as putting 'touch' in a while loop), but it didn't.  
  So far the 'cvs update' is it.
